SepsisCare is a digital platform designed to support survivors of sepsis in their health journey after hospital discharge. Developed in collaboration with Ireland’s National Clinical Programme for Sepsis, it provides structure, guidance, and peer connection during post-sepsis recovery, where previously there was no such support. The platform applies human-centered design and clinical insight to bridge critical gaps in awareness, education, and long-term support.
